Phase I
- Selects at least one EBP based on priority population needs
- Demonstrates understanding of the EBP and how it differs from other practices
- Consults with experts (i.e. CCOEs, others) to implement EBP
- Collects data on EBP impact for people receiving services
Phase II
- Ensures that EBP data are closely monitored by key clinical and administrative leadership
- Fully implements the EBP and uses fidelity measures to identify opportunities for improvement
- Collects EBP client-level outcome data systematically and routinely makes it available to clinicians/practitioners
Phase III
- Tracks impact of EBP at client, practitioner, supervisory and senior leadership levels
- Integrates EBP and its outcome and fidelity data into the agency's formal PI process
- Has developed strategy to sustain the EBP over time
